Financial Matters
Thank you for choosing Moore for Women Healthcare & Wellness for your services; we are committed to providing you with the best possible care.

If you have medical insurance, we are here to help you receive your maximum benefit. Please remember that your insurance is a contract between you, your employer and the insurance company. We will bill participating insurance companies as a courtesy to you but payments and co-payments are required at the time services are rendered.

Bring any required referral for treatment at or prior to your visit. Any referrals given for Injections Medications, Labs, CT Scans, MRI, Mammogram, Pelvic Ultrasound, and Bone Density, etc. will be your responsibility to see if authorization is required prior to making the appointment, however we will assist you.

Billing for Laboratory procedures: All laboratory procedures are billed separately from your office visit. You could receive a laboratory bill from any of the following:
LabCorp: 1-800-845-6167
Quest: 1-800-326-4756

Medical Records
You will need to request in writing for the release of any medical records we have. You authorize us to include all relevant information, including your payment history. Processing of medical records takes 30-60 days and there is a fee.

Surgical
We will verify your insurance coverage before surgery; however, you should also check with your insurance company.

We will submit an insurance claim form. You are responsible for any unmet deductibles or coinsurance payments to be paid in our office prior to the date of surgery. Our Business Office will contact you and outline payment arrangements prior to surgery.
